We have made new patient forms available to you as Adobe Acrobat files. For your convenience, we recommend printing and completing these forms at home. Please remember to bring your completed forms with you to your first appointment. If you have any problems getting files to print properly, please call our office and we will gladly mail or fax them to you.

Your First Visit

When you arrive at our Acton, MA office for your first appointment, you are greeted by our friendly front office team and given some introductory paperwork to complete. An assistant will review your medical history and explain what your exam will include. Feel free to ask questions at any time during this process.

Once you have settled into the treatment area, your periodontist will perform a comprehensive periodontal evaluation, take digital X-rays, and outline a recommended treatment plan. Depending on the plan you both agree to, your care may be completed in one visit or several.

Payment & Financing

For your convenience, you can pay for treatment via cash, check, or credit card (Visa, American Express or MasterCard). We deliver the finest care at the most reasonable cost to patients, therefore payment is due at the time service unless other arrangements have been made in advance. Please remember you are fully responsible for all fees charged by this office regardless of insurance coverage.

We will send you a monthly statement. Most insurance companies respond within four to six weeks. Please call our office if your statement does not reflect insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated.

We also make arrangements for monthly payments via CareCredit, but this must be done prior to the actual procedure. If you have any questions about what your insurance will cover or our policies, please call to speak with one of our team members. A simple telephone call can clear up misunderstandings.
